What is her real name?
Takemura Kiriko (竹村きりこ)

Why is she called Kyaru Pamyu Pamyu?
Kyary was her nickname since high school. She thought the name was too short so added the Pamyu Pamyu after hearing a comedian using the word Pamyu which she thought was cute.

How do you pronounce her name?
Kyary is pronounced Carrie and Pamyu is pronounced Pam You.
